Over Exposure to Radiographer

The licensee radiography crew believed they had retracted the source back into the body of the radiography camera. They then approached the pipe. One radiographer noticed that an indicator on the camera suggested the source was actually not fully retracted into the body of the camera. The radiographers' survey meters registered a zero reading; however, one radiographer's digital rate meter was alarming. One of the radiographers turned one crank on the camera, and that is when the source became fully retracted.

Worker Overexposure

A Nuclear Medicine technologist received an overexposure to the extremity dosimeter for March 2011. The extremity dose was reported as 564.4 mSv (56.44 rem). During the month of March, the technologist performed several cell-labeling procedures involving 2,775 MBq, 4,107MBq, and 3,996 MBq (75mCi, 111mCi, and 108mCi respectively) amounts of Y-90. There were also several labeling procedures involving the use of I-131 during the same time frame.

OVEREXPOSURE TO RADIATION WORKER

On January 13, 2011, the licensee received a dosimetry report which stated that one of their employees received a year-to-date exposure of 55 mSv (5.5 rem) which was above the statutory limit of 50 mSv (5 rem). The overexposure was received in December 2010 during the emergency repair of a cyclotron unit over a period of several days. However, the affected employee’s real-time dosimeter indicated a much lesser exposure, calling into question the measurement indicated on the permanent dosimeter.

During regulatory control carried out on in decommissioned old foundry facility located in LUBLIN (city located near the eastern border of Poland) regulatory inspectors of NAEA discovered lack of 7 lead containers (Type: PrJ20) with Co-60 (non-dangerous source, category 5). The lost or stolen sources contain Co-60 with following activity, on December 7 current year, as follow:
1. 9 MBq
2. 20 MBq
3. 20 MBq
4. 20 MBq
5. 13 MBq
6. 17 MBq
7. 17 MBq
The investigation is being conducted.

Co-60 orphan source discovered in a container of metal scraps in the Genova Port

On July 20th, at the port of Genova Voltri, an elevated gamma rate was detected in corrispondence of a container of metal scraps. Radiometric measures revealed the presence of a Co-60 source, with an hot –spot of gamma dose rate of 600 mSv/h at contact with one wall of the container, about 40 mSv/h at one meter and 100 microSievert/h at 25 meters from the container wall. Moreover, radiometric survey around the container showed that from the other sides of the container the dose rate level was in the range from 20 to 80 microSievert/h.

Member of the Public Overexposure

The potentially overexposed individual may have spent a significant amount of time visiting his fiancée who was receiving a temporary implant of Cs-137 and Ir-192 seeds via low dose-rate remote afterloader brachytherapy on April 16-18, 2010. The licensee instructed the patient's visitor to visit no longer than 2 hours in a 24-hour period and to stay behind the bedside shield during these visitations. On Tuesday, April 20, 2010, the Assistant Nurse Manager informed the licensee that the visitor claimed to have spent the night in the bed with the patient on two consecutive nights.

On April 07, 2010, AERB received a report from a hospital in New Delhi about a patient, a scrap metal dealer with suspected radiation-induced symptoms. It was also informed that the patient is a scrap metal dealer and has a shop in Mayapuri New Delhi. Two AERB officers were promptly sent to the site who confirmed the presence of radioactive material in the scrap shop of the dealer. Their survey also indicated presence of radioactive material in two other shops in the neighbourhood.

Overexposure in the field radiography

The incident has happened during radiography work with Gammamat model TSI-3, containing Ir-192 source with activity at the time 2.6 TBq (70.2 Ci). The technician operating remote crank mechanism was not able to crank in the source to the shielded position. He asked for help company’s radiation protection inspector (RPI). The RPI with the second worker came in the hurry, forgetting to take their individual dosemeters.
