Accident in industrial radiography |
A radiographer was taking several radiographic films to a pipes by the night. In order to be sure that the guide tube was correctly the radiographer went to the tube guide an collimator to fix them. This operation was made by 40 times. Eventually the radiographer touched with his left hand, at least 10 times, the tube guide where the source was unnoticed. Also, two auxiliar staff went to the radiographer position carrying the films to be checked at least by 40 and 20 times. The radioactive source was 3199,5 GBq Ir-192. The event was detected at the end of job.

THEFT OF NUCLEAR GAUGE |
In April 9th, 2003, a nuclear gauge was stolen from a pick up truck while parked in front of owner offices. A dennounce was made by the owner to the National Police. The nuclear gauge was inside its normal transport package. The radioactive sources inside the gauge are 296 MBq of Cs-137 and 1,48 GBq of Am-Be. The radiation level around the nuclear gauge are nearly 50 uSv/h at most.
The nuclear gauge was unauthorized by the regulatory body.

In January 21th, 2002, a portable nuclear gauge was theft from a transport agency where it was stored after being sent from other location. The equipment was removed by another people having false Identification Document. A legal claim was presented by the owner of nuclear gauge to the National Police. The nuclear gauge was inside its normal transport package. The radioactive sources in the nuclear gauge are 296 MBq of Cs-137 and 1,48 GBq of Am-Be. The normal levels around the package are nearly 30 uSv/h.

Finding of abandoned radioactive source in the street |
On September 11, 2000, a radioactive source was found abandoned in the street by a driver of a governmental agency, who immediately notified the National Authority. The staff of the National Authority went to the site and discovered a gamma camera containing an Ir-192 radioactive source. No contamination was found in the gamma device neither in the people close to the gamma device. The rational levels were 5 micro Sv/h on the surface of the gamma device and less than 0.6 micro Sv/h at 1m. The doses to people were negligible.

Radiological accident in industrial gammagraphy with Ir-192 |
On February 20, 1999, a radioactive source of 1,35 TBq Ir-192 was found out of its container by a welding worker in a field where a construction of a new hydroelecticity plant is underway. The worker took the source with his hands and put it into his trouser in the back pocket. In this way the worker and 5 people around him, including four family members, were exposed.
