Multiple non compliance during a transport |
On December 1st, an excepted package of three flasks containing a limited quantity of carbon 14 (500 micro L for each and a total activity of 1308 MBq) was sent to the German research laboratory "Sanofi Aventis " of Frankfurt. The consignee discovered that one of the flasks was not properly screwed and had leaked in the plastic bag which contained it. Fortunately the package was not contaminated. The leakage was not the only problem noticed: the transport document mentioned only one flask instead of three.

EVENT DESCRIPTION:On July 5, 2006, a type A package containing radioactive material for radiopharmaceutical use has been lost during its transport between Saclay (France), Brussels airport (Belgium) and the consignee located in Balearic Islands (Spain).This package consigned by CIS bio international (France) contains a limited quantity of Mo 99 (a total of 52 GBq).

Lost of type A radioactive package during transport from Brussels to Bucharest |
On 24 July, a type A package containing radioactive material has been lost during its transport between Brussels and the consignee (IFIN-HH)in Romania.
This package consigned by MDS Nordion Belgium contains a limited quantity of Iodine-131 (a total of 222 GBq).
The package has been lost by Austrian Airlines on Austrian airport. At this time the package has still not been found by Austrian Airlines.
Due to the loss of this package, the competent authority of Romania rated this event at level two on INES scale.

Lost radiopharmaceutical during its transport |
On 12 May, a type A package containing radioactive material for radiopharmaceutical use has been lost during its transport between Saclay (France) and the consignee in Tunis (Tunisia).
This package consigned by CIS bio international (France) contains a limited quantity of Iodine-131 (a total of 5.15 GBq).
The competent authority of France has informed the comptetent authority if Tunisia.
Due to the loss of this package, the competent authority of France rated this event at level one on the INES scale.

Non-compliance with regulations during transport of radioactive material |
On 22 June 2004, a transport of radioactive material from the Laboratory for Solid State Physics (LSSP) of the Swiss Federal Institute of Technology Zurich to the Paul Scherrer Institute (PSI) in Würenlingen (Switzerland) was performed. The LSSP was the consignor and the PSI the consignee. The PSI, which is experienced in transporting all types of radioactive material, had the function of the transport coordinator with main transport responsibility. The transport was carried out by a licensed company and escorted by a responsible PSI staff member.

Loss of an excepted package shipped to Slovakia |
End October 2004 the French authorities were notified that the package had arrived on September, 22 to Warsaw airport (Poland) due to an error in the transfer in Praha airport. The package was due to return to France as soon as possible, after having received the appropriate transport authorisation.

No proactive response outside UK expected to be required.
The package (gauge) was found with the sources in place and undamaged. Tests were carried out to confirm that there was no leakage or contamination.

Loss of a type A package in France |
The French Regulatory Authority was informed on 17 September that a type A package containing samples has been lost on 9 September during its transport in France, before intended shipment to the USA.
This package consigned by EURODIF (France) contains two samples of uranium hexafluoride (a total activity of 1.1 MBq). The package, a yellow box 20cm square, has a white labell with number UN 2978.
Due to the loss of this radioactive package, the competent authority of France rated this event at level one on the INES scale.

On July 17, an excepted package containing radioactive material has been lost during its transport between Saclay (France) and the consignee located in La Habana (Cuba).The French authority was informed on July 30 of this event. Due to the estival period, the loss discovering has taken more time than in normal period. This package consigned by CIS bio international (France) contains a limited quantity of Iodine 125 (a total of 1.43 MBq). The competent authority of France informed the competent authority of Cuba.
