Fire in emergency diesel generator exhaust

On October 9, 1997, Limerick 2 experienced a fire in the exhaust stack of the D21 emergency diesel generator (EDG) during an operability test. The unit was operating at 100% power at the time of this event. During the test run, the #4 cylinder lower piston seized, resulting in damage to the piston, connecting rod and cylinder liner. The fire occurred due to the release of unburnt fuel into the EDG exhaust manifold and exhaust stack. The fire burned out when the EDG was secured. A second, smaller, fire occurred on the body of the EDG in the vicinity of the exhaust manifold.

Fire incident in boiler room

While monitoring boiler room through CCTV during plant start-up, flames were observed in the vicinity of one of the main coolant pumps. Reactor was immediately shut down, and primary heat transport system crash cooled to 300 deg. F. Shift fire fighting crew rushed to the affected area to extinguish the fire. At the fire site it was observed that the insulating material of the piping close to the main coolant pump was burning.

Power excursion and subsequent plant trip

Due to problems in the Process Salt water heat exchangers, the performance of control room air-conditioning chillers (which is cooled by the process salt water) was affected, resulting in rise of the control room temperature (up to 87.5 deg. F). this rise in control room temperature adversely affected the satisfactory operation of the two regulating computers. At 0153 hrs controlling computer B started internal initialization frequently, while controlling computer A stalled at 0200 hrs. without any alarm.

Unit trip on no primary circulating pump running

On 19.03.96, unit was operating at 210 MWe. At 1627 hrs. bleed condenser got boxed up due to spurious signal of high bleed cooler outlet temperature. Bleed condenser level started rising, which initiated reactor set back. Reactor power started coming down and turbine tripped on reverse power. Running feed pump tripped on bleed condenser pressure high. This caused tripping of all primary coolant pumps resulting in reactor trip. The event is rated at level 0.

Reactor trip on PHT pressure less than 79.6 kg/cm2 for 15 seconds

On March 02, 1997 monthly Emergency Core Cooling System (ECCS) test was in progress. While carrying out the injection test, fluctuation in the Primary Heat Transport (PHT) system pressure was observed. Subsequently PHT system pressure went up to 89 kg/cm2. During this transient bleed flow increased steeply due to bleed control valve stuck open for about 80 secs. To cope up with bleed flow, feed control valves also got opened. Reacor set back got initiated due to bleed condenser high level. This was flollowed by reactor tip on PHT system pressure low.

Loss of the residual heat removal system during refueling

The plant was in mode 6 - refueling. Nuclear fuel was in the reactor vessel, reactor vessel head was removed and the reactor cavity was flooded. In accordance with the Technical Specifications one train of the Residual Heat Removal System (RHR) was in operation.

Fire involving used extract ventilation filters

A number of ventilation filters set alight causing minor fire (outside building 633, enriched Uranium Chemical facility, formerly EURRP) which was brought under control by the sites own emergency services within about half an hour. Lancashire County Fire Brigade were called out as a precautionary measure and assisted in dampening down the fire. Nobody was hurt and there is no effect on members of the public. An investigation is being set up to establish the cause of the incident.

Leakage on thge feedwater recirculation pipeline of the steam-generator no. 1

The reactor was operated at 420 MW and 4 loops. At 17:11 p.m. on 23.03.97 during routine surveillance the operator discovered steam in the steam generator box no. 513/1. The primary circulation pump has been switched off by hand, automatically the reactor power has been decreased to 310 MW and steam generator no. 1 has been drainaged.
At 18:20 p.m. on 23.0397 reactor power had been recovered to 400 MW with 3 loops.
At 10:25 a.m. 24.03.97 reactor power had been recovered to 420 MW with 3 loops.

Reactor trip due to short circuit on control and protection system of generator

On 4 February, 1997 the plant was under 365 MW electrical power operation with two working turbogenerators. At 6:31 a.m. one of the turbogenerators was disconnected from the grid and tripped by the control and protection system. The reactor power was decreased in order to maintain the parameters of primary and secondary circuits. At 6:34 the reactor was shut down by the control and protection system because of drop of level in the steam generators 1-3.

Inadvertent closure of the MSIV caused safety injection and plant trip

At 08:33:30 a.m. the Main Steam Isolation Valve (MSIV) no. 20142 on Train B inadvertently closed thus isolating Main Steam Train B. As a result the Train A steam flow increased and the steam pressure dropped below the setpoint for Safety Injection which initiated safety injection and reactor trip. The reactor trip sequence proceeded in accordance with the design, all safety systems were available. The plant was stabilized at "no load" parameters. The first investigation which concentrated mainly on instrumentation and control circuits of the valve revealed no irregularities.
