Event during operation of Zaporizhzhya NPP Unit №3

On 28 of November 2014 at 19:24 the disconnection of the Unit №3 of Zaporizhzhya NPP from power grid occurred. According to the preliminary information the cause of the occurrence was the failure of electrical equipment in the turbine hall.
As of 3rd December 2014 the reactor facility remains in the “cold” shutdown.
According to the preliminary evaluation this situation is qualified according to INES scale – “0-deviation (No Safety Significance)”.

Discovery of damaged fuel rods during core unloading and fuel inspections

In October 2013 the refuelling outage started in the Krško NPP. During transfer of fuel assemblies from the reactor to the spent fuel pool a 50 cm long segment of a fuel rod was found at the bottom of fuel transfer channel. Visual inspection of all fuel assemblies unloaded from reactor core showed open defects in 9 fuel rods cladding of 4 fuel assemblies. These 9 fuel rods were all broken with some parts of cladding and fuel pellets missing. The cause of such extensive fuel cladding failure is secondary degradation by zirconium hydride.

Alert Emergency Action Level Declaration due to Loss of Offsite Power Resulting from a Seismic Event

On 23 August 2011, Units 1 and 2 of the North Anna Power Station declared an Alert, Emergency Action Level due to a loss of offsite power following a 5.8 seismic event on the Richter Scale. As of August 24, 2011, Unit 1 and Unit 2 are proceeding to cold shutdown conditions to allow for inspections inside containment. Normal offsite power has been restored to the North Anna Power Station.

At 1403 (EDT), on August 23, 2011, Units 1 and 2 of the North Anna Power Station declared an Alert, Emergency Action Level due to a loss of offsite power.

Hydrogen Recombiner functional testing not satisfactory

On January 24, 2011 the Laguna Verde NPP Unit 2 was operating at 21% of rated power. While testing the Hydrogen Recombiner System Division I, it was observed that there was not an increment in the temperature of its heater; therefore the test was not satisfactory. All other safety systems were at operative condition.

Reactor trip due to failure in the Main Generator

On January 15, 2011, Laguna Verde NPP Unit 1 was operating at 37.3 % rated power and in the process of decreasing power in order to repair a component related to the electrical protection of the main generator. Suddenly there was an automatic reactor trip due to the failure of this electrical protection. All safety systems were in operative condition and worked as required, and the reactor was taken to a safe shutdown condition.

Unidentified leakage from the primary circuit in excess of TS limit

The Krsko NPP was safely shut down on 4 June 2008 at 8:10 p.m. (local time), after a primary circuit leak was detected at 3:07 p.m. on the same day. The leak of about 3 m3/h has exceeded the limit of the plant Technical specifications and required the plant to shut down. The event was classified as an unusual event. There was no demand on the safety systems. The loss of coolant was controlled by the charging flow. There was no need for off-site protective measures since there were no releases to the environment.

TURBINE TRIP AND MANUAL SCRAM DURING NORMALIZATION OF A HEAT EXCHANGER OF THE MAIN OUTPUT TRANSFORMER

On December 15th, 2007, while putting in service a heat exchanger of the main output transformer, after its reparation, the actuation of the protection of the block occurred, producing a turbine trip and electric commutation on the start up line. After that a manual SCRAM was needed according to the operating instruction, not being able to normalize the Electric System in the required time.

On September 10, 2007 the VROM Inspectorate was notified by te Customs Office at Rotterdam Maasvlakte that they had a radiation alarm of their portal detector. By mistake the content of the container was already unloaded at a distribution center in Amsterdam. Using handheld survey equipment an enhanced radiation level at the surface on the unloaded boxes of 15 microSieverts per hour could be determined. The container identification number is PONU 73766-5. The container was completely filled with 824 ladies handbags.

Events related to the earthquake on July 16th

Detailed investigation on all power reactors (units from #1 to #7) is still underway at Kashiwazaki-Kariwa nuclear power station. Nuclear and Industrial Safety Agency (NISA) of Japan, on November 13th, finalised the INES ratings for the four incidents which are found after the earthquake occurred on July 16th 2007. Other events including the bended ventilation duct found at Units #1 to #5, and water spillage on B5th floor of the reactor complex building of Unit #1 have been repaired and are considered to impose no risk of radioactive materials leakage.
