Error in operating rules leading to safety injection system unavailability

When entering a shutdown state at the Dampierre plant, the Safety Injection System was made unavailable due to the application of an erroneous operating rule, which does not permit to fulfill the requirements of technical specifications. This rule had been written by the Tricastin Plant and was also used as it was (i.e. with the error) by Dampierre and Bugey plants.

This incident occurred several times on these three 900 MWe plants during the following years.

Due to lack of safety culture (deficiency of the quality insurance process) this incident is rated at level 2 according to the INES scale.
