NEW FUEL ASSEMBLY DROPPING WHILE REFUELING

While loading three new rejected fuel assemblies (FAs) into the shipping cask the rupture of the hoist cable with subsequent FA dropping occurred, which resulted in new FA damage. No damage to fuel rod cladding and no radioactivity releases occurred. The cause of cable rupture was the failure of the hoist end switch.
BASIS FOR RATING: Initiator - real, possible (new FA dropping). Safety function availability - full. According to Table A2 level 1 (A-2R) is assigned. Event cause reveals safety culture deficiencies (deficiencies in organizational-technical measures for FA transportation) and therefore the level higher by 1 has been chosen.
