PRIMARY WATER SPILLAGE WHILE REPAIR OF THE GATE VALVE AT RCP SUCTION DURING SCHEDULED MAINTENANCE

The unit was in the scheduled maintenance outage. Reactor was shut down. While repairing the gate valve at RCP suction the rubber-metallic plug from the suction header side was pressed out by water column. As a result of draining of the downcomer section of the primary circuit right side the water got into the rooms adjacent to RCP sealed room.
Personnel exposure and radioactivity releases to environment above levels prescribed for normal operation did not occur. Event causes are under investigation. Breach of safe operation limits and conditons did not occur.
BASIS FOR RATING: Initial event - primary water spillage. Event - expected, real. Safety function availability - full, which corresponds to level 1/2 (A-2R). Since the potential cause of the event in safety culture deficiencies the maximum level has been chosen.
