ISOLATION OF FUEL OIL TO ESSENTIAL GAS TURBINES

During test of a gas turbine it was discovered that fuel oil supplies to all gas turbines had been inadvertently isolated 18 hours earlier. Fuel oil supplies were restored within one hour.
BASIS FOR RATING: Safety function availability: adequate as in the event of loss of grid, several hours would have been available to restore supplies.
Postulated initiator: loss of grid.

Other factors: Required operator actions could not be carried out form control room. Evidence of safety culture/procedural inadequacies. Period of unavailability was short compared to loss of grid frequency.
